Severe intra-abdominal haemorrhage: a consequence of two coinciding events.

چکیده
Idiopathic thrombocytopenic purpura, an immune-mediated disease, usually has a relatively benign clinical course. Bleeding manifestations are mostly mucocutaneous and mild. Massive haemorrhages requiring transfusions or other interventions are rare, unless platelet counts are extremely low or other complicating conditions coexist. The rupture of an ovarian follicle is a very common benign condition in women of the reproductive age group. Any associated bleeding is unlikely to be significant, unless there is an underlying severe haemostatic derangement. We describe a 24-year-old Filipino woman presenting with severe abdominal pain and intra-abdominal haemorrhage requiring laparotomy, which revealed massive haemoperitoneum and a ruptured ovarian follicle. She had thrombocytopenia secondary to previously undiagnosed idiopathic thrombocytopenic purpura. This case illustrates how a combination of two otherwise common and benign clinical entities can result in a rare and potentially life-threatening event.

Increased endometrial vascularity and intraoperative haemorrhage among women undergoing surgical evacuation for retained product of conception
The aim of this study was to ascertain the incidence of "increased endometrial vascularity" recorded in the ultrasound scan reports, in the context of RPOC and correlate this finding with the incidence of life threatening intra-operative bleeding. Ultrasound scan reports sometimes mention the presence of increased endometrial / sub-endometrial vascularity in the context of retained products of ...

The following case of severe intra-peritoneal haemorrhage from a corpus luteum is of considerable interest at this time, as gyntecologists are now again recognising that bleeding into the pelvis may be due to other causes than rupture of an ectopic pregnancy. I am inclined, from my experience in abdominal surgery, to think that a certain degree of haemorrhage into the peritoneal cavity, ovarian...
متن کاملUltrasound Imaging in A Case of Ruptured Hydatid Cyst Complicating Pregnancy – A Case Report
Ruptured hydatid cysts are rare causes of intra-abdominal haemorrhage. They are usually secondary to primary hepatic hydatid disease. They are even rarer causes of intra-abdominal haemorrhage in pregnancy. Untreated cases or cases with delayed diagnoses are associated with a poor outcome due to systemic hypotension and secondary organ damage resulting from exsanguinating haemorrhage. Ultrasound...
